Bilateral Bandage Contact Lens Treatment in a Boston Keratoprosthesis due to a PAX6 Gene Mutation

Abstract

Purpose: The aim of the case report is to describe the Benefits of bandage lenses (VBL) in a patient with a PAX6 gene mutation and a Boston keratoprosthesis in both eyes in combination with extended contact lens wear in high-risk eyes.

Material and Methods: The anterior segment of the eye was examined using a Haag Streit Visutron 900 touch projector, the Oculus visual acuity chart, measuring glasses and lenses, and a Haag Streit BQ 900 slit lamp with camera unit. Pathological findings of the anterior segment of the eye were classified using the JenVis classification key. The “Individual” lens type from Galifa (St. Gallen, Switzerland) was fitted as a bandage lens.

Results: Examination of the anterior segment of the eye showed among others a watery tear film in both eyes, a conjunctivalized cornea up to the pupil diaphragm, and severe bulbar conjunctival hyperemia of the conjunctiva. The Bandage lenses with refractive correction were well tolerated – the visual acuity could be increased in both eyes.

Conclusion: Keratoprostheses have proven to be a successful treatment option for high-risk eyes. Postoperatively, the wearing of a bandage lens in conjunction with a keratoprosthesis is necessary. The soft contact lenses fitted in this case were well suited for the treatment of high-risk eyes due to is customized design.
